- When registering as a patient:
- Medicare number;
- referral documents from health professionals;
- physical and mental health history;
- medications and other relevant health details.
- During consultations:
- sensitive information relevant to the assessment or treatment of your mental health;
- detailed psychological and physical health information;
- other sensitive information you choose to share during sessions.
